Immunization Forms
Please download the PDF forms from the links below. Upload the completed forms here after logging in.
In accordance with the New York State Department of Health, the following forms are required for all enrolled students at LIU.
  • Measles, Mumps, and Rubella (MMR)*
  • Meningococcal Disease (Meningitis)*
In addition, the following forms are strongly encouraged for students to complete:
  • Health Assessment Form
  • Medical Consent Form for Minors (if applicable)
